We would like to take this opportunity to welcome you to our school.  West Elementary is a growing school and has been providing quality education since 1959.  Our principals, teachers, and staff are dedicated to providing every child with the best educational opportunity possible. 

Below is a list of the requirements for kindergarten enrollment.  Please keep in mind that we must have all of the items turned in by the first day of school.

_____  Age:   A child must be five years of age by September 30^th to enter kindergarten.

_____ Certified Birth Certificate: An official copy of the child's birth certificate is required.  If you do not have an original, please visit your local health department or call 615-741-1763.

_____ Social Security Card:   An official copy of the child's social security is required.  If you do not have a social security number, please call 800-772-1213.

_____ Immunizations and Physical:Proof of immunizations and physical must be on a Tennessee form provided and signed by your doctor or health department official and current.

_____ Proof of Residence:   Each child must have two of the following…

…Utility bill such as an electric or water bill

…Signed lease agreement

…Property tax statement

…Driver's license with current address

…Voter registration

…Recent paycheck with name and current address
