Necessary Entry Documents

 

 

Tennessee state law requires that children entering school must have a Tennessee Child Health Record.  This record must be completed with all the required immunizations as well as a health examination.  Both sections of the record must be signed by a medical provider which indicates that the child is in compliance with the requirements necessary for enrollment.

 

 

Tennessee Permanent Immunization and Medical Form A State of Tennessee approved form must be signed by a doctor in two places and completely filled out with the following information.

1.  DTP/DTaP/DT/Td - 4 or 5 doses, one dose must be given after age four (4)

2.  POLIO – 3 or 4 doses, only 3 doses if 3rd dose is given after age four (4)

3.  HEPATITIS B – 3 doses

4.  MMR – 2 doses given at 15 months, 2nd dose at least 30 days later

5.  VARICELLA (chickenpox) – 1 dose given after 1st birthday or history of disease

6.  HEALTH EXAMINATION required before entry.

 

 

OTHER DOCUMENTS NECESSARY PRIOR TO ENROLLMENT

 

Birth Certificate

 

Proof of Residency – Example: lease agreement or utility bill (not phone).

 

Social Security Card – For administrative purposes only.

 

Custody Papers – If a child is not living with both biological parents, or if a child lives with only one parent, proof of legal guardianship or custody papers is required and must be singed by a judge.
