NECESSARY ENTRY DOCUMENTS

PRIOR TO ENROLLMENT YOU MUST HAVE THE FOLLOWING DOCUMENTS

c Tennessee Permanent Immunization and Medical Form
Green form signed by the Doctor in two places and completely filled out.

c Birth Certificate

c Proof of Residency Example: Electric Bill, Water Bill, with your name and address appearing on that bill. Driver’s License or Addressed check will not fulfill this requirement.

c Social Security Card. ( For Administrative Purposes)

c Custody Papers If a child is not living with parents, or if a child lives with only one parent, proof of legal. Guardianship or custody papers are required.  These must be signed by a judge.
