Leaving for Doctor's Appointments |
Parent/Guardians: Please make every effort to make your appointments after 12:00 pm or preferably after school hours. |
Steps for going home sick from LHS |
1. Students who are sick or injured should go to see the Nurse. 2. The Nurse will assess the student's health and make a determination about whether the student should go home or stay at school. 3. If the student actually needs to go home, the Nurse will contact their Parent/Guardian using the numbers from their School Heath Form / Computer Contact. 4. If the Nurse calls and there is no answer, the Nurse will leave a message and then try another contact number. 5. If the student needs to go home, then a parent/guardian must come and sign the student out. IDENTIFICATION WILL BE REQUIRED to sign an student out. 6. If a student brings a note or a note is faxed to the school from the student's parent/guardian, then the student may sign out. |
In order for your child to register for classes at Lebanon High School, he/she must have the following information: |
IMMUNIZATON RECORDS |
If transferring from a Tennessee school this form should already be on file and will be transferred when official records are requested by us. If transferring from private, out-of-state or homeschool, a Tennessee Certificate of Immunization (green form) will be required. This can be obtained through the Wilson County Health Department (located in Lebanon, TN), a local clinic, or doctor's office. First time students in this state must have proof of physical examination not more than one year old. |

RECOMMENDATIONS |
Special Conditions apply to the following: |
All students should be kept home and by symptom free for at least 24 hours before returning to school if they have the following symptoms: 1. Temperature of 100 degrees or more. 2. Yellow drainage from eyes and/or matting of eyes, which could indicate pink eye. 3. Vomiting and/or diarrhea. 4. Sore throat, stomachache and headache accompanied by swollen glands around the jaws, ears and neck. |
Chicken Pox - all eruptions must be scabbed over before returning to school. Skin conditions such as scabies and impetigo must have verification from a physician that they are no longer contagious before returning to school. Students diagnosed with head lice must have been properly treated and must be brought to school by a parent/guardian to be checked by school personnel before returning to school. Our policy is that the student must be lice and nit free. |
