MT. JULIET SENIOR HIGH SCHOOL
REGISTRATION REQUIREMENTS FOR NEW STUDENTS
                                                                                                       

The following requirements must be met prior to enrollment.
Transfer students must have withdrawal form from previous school.

 Academic Records – Grades and credits from previous school.

-  If enrolling at the beginning of the school year this can be an unofficial transcript, or  a report card.  During the school year, this should be a withdrawal form and unofficial transcript.

Immunization Records –

-  If transferring from a Tennessee school this form should already be on file and will be transferred when official records are requested by us.
-  If transferring from private or out-of-state a Tennessee Certificate of Immunization (a green form) will be required.  This can be obtained through the Wilson County Health Dept. (located in Lebanon, TN), a local clinic, or doctor’s office.
-  First time students in this state must have proof of physical examination not more than one year old.

Proof of Residency – two of the following required: (two proofs of residency)

-A utility bill or hook-up verification from either the gas, water, or electric companies reflecting name and address.-Copy of a signed lease agreement or settlement statement.
-Copy of current bank statement.
-Voter registration card, and photo ID (driver’s license or passport).
Custody Papers (If student does not live with natural parents)
Court records, signed by a judge must show proof of legal custody or guardianship by the person the student is living with.

Student Data
Social Security Number, Date of Birth, Driver’s License.
