Procedure for students going home sick
- The
sick student comes to the Nurse’s office
- The
Nurse will assess the student and when necessary call the
parent/guardian from the Nurse’s office

If a student comes to
the Nurse’s office and the Nurse is out, the Front Office can and
will always get in touch with the Nurse

In order for your child to register for classes at MJHS,
he/she must have the following information.
Immunization Records
If transferring from a Tennessee school this form should already be on file and will be transferred when official records are requested by us.
If transferring from private or out-of-state a Tennessee Certificate of Immunization (a green form) will be required. This can be obtained through the Wilson County Health Dept. (located in Lebanon , TN ), a local clinic, or doctor’s office.
First time students in this state must have proof of physical examination not more than one year old. |

Special conditions apply to the following: |
All students should be kept at home and be symptom free for at least 24 hours
before returning to school if they have the following symptoms.
- Temperature of 100 degrees or more
- Yellow drainage from eyes and/or matting of eyes, which could indicate pink eye.
- Vomiting and/or diarrhea
- Sore throat, stomachache, and headache accompanied by swollen glands
around the jaws, ears and neck.

Chicken Pox - all eruptions must be scabbed over before returning to school.

Skin conditions such as scabies an impetigo must have verification from a physician
that they are no longer contagious before returning to school.

Students diagnosed with head lice must have been properly treated and must be brought
to school by a parent/guardian to be checked by school personnel before returning to school.
Our policy is that the student be lice and nit free.
