Leaving for Doctor's Appointments
Parents/Guardians: Please make every effort to make your appointments after 12:00 pm or preferably after school hours. |
Steps for going home sick at MJHS
Students who are sick or injured should go to
the clinic and see the Nurse.
The Nurse will assess the student’s health and make a determination
about whether the student should go home or stay at school.
If the student actually needs to go home, the Nurse will call their Parent/Guardian
using the numbers from their School Health Form.
If the Nurse calls and there is no answer, the Nurse will leave a message and then try another contact number.
If the student drives to school the Nurse, along with the Parent/Guardian,
will determine whether the student is well enough to drive themselves home.
If the student doesn’t drive, the Parent/Guardian must come to school and sign them out. |
In order for your child to register for classes at MJHS,
he/she must have the following information.
Immunization Records
If transferring from a Tennessee school this form should already be on file and will be transferred when official records are requested by us.
If transferring from private or out-of-state a Tennessee Certificate of Immunization (a green form) will be required. This can be obtained through the Wilson County Health Dept. (located in Lebanon , TN ), a local clinic, or doctor’s office.
First time students in this state must have proof of physical examination not more than one year old. |
