School Registration

To register for kindergarten, out-of-state, or coming from private school:

 

1.  Certified birth certificate (not the mother’s or hospital copy). A child entering kindergarten shall be no less than five (5) years of age on or before September 30 of the current school term or by December 31 if he/she transfers from another state.  You can order certificates from http://www.vitalchek.com

 

2. Tennessee immunization and physical examination form (these may be obtained at the health department or doctor’s office)

 

3.  Two proofs of residence (such as: current utility bill with the home owner’s name and address - electric, natural gas or water bill)

 

4.  Notarized letter of residence (if applicable) if you are living with a friend or relative who is the homeowner of your place of residence

 

5.  Proof of custody (if applicable) – If a child is not living with his/her parents or if a child lives with only one parent, proof of legal guardianship or custody papers are required. These must be signed by a judge. (We do not accept notarized forms.)

 

6.  If your child is transferring from another Tennessee public school, all of the above mentioned items (except the birth certificate and the social security card are required for registration).  Any child residing within the geographic portions of Wilson County may attend the public schools of Wilson County, but must attend the school specified for that county school zone unless otherwise approved by the Board or its designee. Legal residence is defined as the primary domicile of the student’s legal parent/guardian.

 

Medical Requirements:
*DTP-DtaP-DT-Td 4 or more doses
, one of which must be given on or after the fourth birthday. If starting series at age seven or older, only three doses are required.

 

Polio (OPV/IPV) 4 doses of OPV or IPV. If the 3rd dose was given on or after the fourth birthday, the 4th dose is not required. However, if a combination of IPV/OPV (2IPV/2OPV) is used, all 4 doses are needed regardless of age.

 

MMR 2 doses , given on or after the first birthday, required for entrance into grades K, 4, 8, and 12. The second dose should be administered at least 30 days following the first dose.

 

Hepatitis B 3 doses , for entrance into Kindergarten.  Required immunizations are defined as those specified by law. However, most health departments and physicians would recommend more than the required doses.

 

Varicella (Chickenpox) - For kindergarten only, required on or after the 1st birthday; may be given no earlier than 4 days before the 1st birthday.  Parental or physical diagnosis of chickenpox also meets this requirement.
