• Students that are planning to take their driver’s permit assessment or take their driver’s license course, must request an attendance verification form from Ms. Ables. This document is required in order for students to obtain their permit or license.

    The form must be requested prior to the permit or license appointment. Ms. Ables will not process same-day requests for the forms. In addition, the forms are only valid for 30 days. After 30 days, a new form must be requested.
